As a Home Health Clinical Manager, you will orchestrate and supervise all direct patient care services provided by our clinical team.
*Job Details:* Role is located in Bradenton, FL in an office setting.
Work schedule is typically Monday to Friday 8AM-5PM.

*Job Description Summary: * Your responsibilities include developing, planning, implementing, and organizing clinical operations at our specific location.
You will be responsible for observing and assessing the delivery and quality of client care and providing leadership and guidance to nursing staff to enhance performance and maintain standards of excellence.
*Role Description: * Clinical Responsibilities: · Conducts/delegates the assessment and reassessment of patients, including updating of care plans and interpreting patient needs · Manage caregiver assignments and oversee the delivery of care to all patients.
· Receives case referrals.
· Make admission decisions based on patient needs and ensure compliance with admission criteria.
· Attend supervisory visits with clinicians.
· Lead quality improvement activities to address identified areas for enhancement.
· Provide direct patient care in emergencies Leadership Responsibilities: · Supervise approximately 15 clinical staff, including hiring, training, and performance evaluation.
· Provides leadership and guidance to assigned nursing staff.
· Instruct, assist and guide clinicians to enhance performance and ensure quality home care services.
· Review and ensure appropriateness of clinical documentation.
· Monitor documentation compliance with regulatory agencies and third-party payers.
· Coordinate communication between team members, attending physicians, and caregivers.
· Collaborate with the Branch Director and Company Finance Department to establish revenue and budget goals.
· Participate in sales and marketing initiatives.
· Conduct staff education based on clinical documentation review and feedback.
· Analyze situations, identify problems, and implement performance improvement initiatives.
· Act as Branch Director in their absence.
· Participate in performance improvement activities and maintain ongoing clinical knowledge.
· Maintain relationships with referral/community sources and participate in professional organizations.
*Required Experience & Skills:* o Two years of recent experience as a Registered Nurse in home care or hospice environment.
o Minimum one year of recent experience in a supervisory role, preferably in healthcare settings.
o Graduate of an accredited School of Nursing.
o Current state license as a Registered Nurse.
o Minimum one year hands on OASIS experience o Ability to effectively lead and motivate a team.
o Proof of current CPR certification.
o Valid driver’s license, auto insurance, and reliable transportation.
*Preferred Experience & Skills:* o One year of Homecare Homebase o One year of quality assurance experience o Strong understanding of quality improvement principles and methodologies.
